Immunizations

Immunization records are requested during the admission process and must be maintained during the student’s academic career at Coram Deo Academy.   Please see Health and Safety section of Parent/Student Handbook for more information. Requirements are also shown in "Health Forms" on the left of this page.

Medication Administration

In order to administer any kind of medication to students, school administration needs permission from parents via the medication administration form. To view this form please view Health Forms through the link to the left.
